How can local communities deal with public health issues and care needs of community members? How can we support them to develop their models of caring for their people? How can we foster the engagement of people and communities as co-producers of care? These questions will lead the first Transnational Forum on Integrated Community Care (Transform) conference in Hamburg on September 24 to 26. This initiative, supported by the Fund Dr. Daniël De Coninck, managed by the King Baudouin Foundation, will bring together practitioners, policy makers and experts around this new and complex issue.
Today, the health care system is fragmented and disease-centred, difficult to navigate and do not consider the person as a whole. TransForm aims to put the community at the centre of primary care and integrated care. It recognizes the capacity of local communities to deal with public health issues and care needs of community members. The overarching aim of the project is to trigger the interest of and inspire policy-makers and practitioners to foster integrated community care through conferences, site visits and case studies dissemination. The ultimate goal is to mobilize change at policy and practice level.
The project foresees a series of 4 transnational conferences that extend until 2021, in order to analyze ‘promising practices’ in integrated community care and to learn from their experiences and impact. The conference in Hamburg is the first in this series. Around 80 participants, among whom practitioners, policy makers and distinguished experts will convene to discuss and explore the potential of integrated community care. Next to presentations and debates, participants will have the opportunity to visit inspiring local initiatives.
The Transnational Forum on Integrated Community Care is a joint initiative of 7 Foundations in and beyond Europe: Fund Dr. Daniël De Coninck (managed by the King Baudouin Foundation), Fondation de France (France), Robert Bosch Stiftung (Germany), la Caixa Foundation (Spain), Compagnia di San Paolo (Italy), Graham Boeckh Foundation (Canada) and the King Baudouin Foundation (Belgium). It is hosted by the Network of European Foundations.
